Willow Foundation is a Pennsylvania nonprofit organization. This Privacy Policy describes how Willow Foundation collects, uses, and protects information provided by members, participants, donors, volunteers, and website visitors.
Information Collected
Willow Foundation collects information provided directly through website contact forms (name, email address, and message content), through intake and enrollment at Heartwood Center, Alderwood Center, and Willow Counseling, and through donation and volunteer processes. Willow Foundation does not collect phone numbers through its website.
Information collected through program intake may include contact information, demographic information, service history, and other information necessary to provide services. Program records are maintained in accordance with applicable state and federal requirements, including 45 CFR Part 164 (HIPAA) and 42 CFR Part 2 where applicable.
How Information Is Used
Information is used to provide services, communicate with members and participants about their care, coordinate program operations, respond to inquiries, process donations, and meet legal and grant reporting obligations. Information is not sold, rented, or shared with third parties for marketing or promotional purposes.

Willow Foundation shares information only as necessary to provide services, with the consent of the individual, or as required by law. Program information subject to HIPAA or 42 CFR Part 2 is disclosed only in accordance with those regulations and the individual's written authorization.

Opt-out. Reply STOP to any message to unsubscribe. Consent may also be withdrawn by notifying any Willow Foundation staff member verbally or in writing.
