  • Meal Services

    Willow Foundation provides access to breakfast and lunch served daily to any of our guests at the Heartwood Center. Our meal services focus on dignified and equitable access to nutritious food regardless of disability, housing status, and dietary restrictions. 

    We also maintain an on-site food pantry. We partner with Commission on Economic Opportunity (CEO) Weinberg Food Bank as well as Midwest Food Bank which stock our basement storage space with nonperishables. We also maintain a partnership with Food Dignity® which provides access to locally sourced, farm-fresh produce, protein, and dairy for our clients.

  • Case Management Services

    Willow Foundation’s Heartwood Center employs two full-time case managers funded by Luzerne County Office of Mental Health and Developmental Services (MHDS). The case managers develop individualized goal plans, facilitate skill-building, provide a series of coordinated interventions and act as an advocate and liaison to community-based resources. The case managers work in tandem with trained support staff and volunteers at to ensure quality service delivery. Support staff includes health and wellness workers who prepare meals, mitigate safety and health risks through proper cleaning and maintenance protocols, provide direct material assistance, and engage with individuals through modeling positive social interactions.
